Hospital Liaison Support for Carers

When a loved one leaves hospital after a stay, to return home, it can be a daunting time. Our Hospital Liaison Worker, who works in partnership with Borders General Hospital, is there to ensure this transition is as smooth as possible. She visits the wards and holds carer one-to-ones regularly in order to fully understand your personal circumstances. She will then work with you intensively whilst the patient is in hospital, and two weeks after discharge, to make sure all the support and services are in place and you are ready, and prepared, to welcome your loved one home and continue your caring role in a healthy, happy way. After this time, you will be allocated your own named Carer Support Worker who will work with you as long as you require them.

The Hospital Liaison Worker will also support you through any choices or decisions you may need to make regarding residential care homes, or the process of palliative/end of life care and choices.
