Supporting Your Recovery (Home From Hospital)

The Supporting Your Recovery service aims to help older people who are in hospital due for discharge to transition back home with the support of our volunteers. Particularly those who live alone and have little or no support network. This is a 12 week service in which we aim to set achievable goals with the client to become more confident, independent and part of their local community. This can include encouragement to take the bus, to become more mobile by going out for a walk, going to a social group, help with shopping, transport to appointment. Goals are specific to each individual. We also signpost to other services if appropriate. The welfare of our clients is priority and our volunteers always conduct a Safe Warm and Well check as well as raise any safeguarding concerns. Regular reviews are conducted during the 12 weeks.
