Inverclyde Health and Social Care Partnership
Inverclyde Health and Social Care Partnership (HSCP) is a new organisation that builds on our previous CHCP arrangements. It formally brings together community health services and social work services. This means that if you need support from either or both, you can have an assessment done that will make sure you get the right help, through joined up teams.
For example, Nurses work closely with Home Helps; Health Visitors work closely with Social Workers; Mental Health staff work closely with the Advice Team – there are all kinds of combinations.
The main point is that the right services are working together to get the right results for people. The HSCP will work to improve lives by focusing on the outcomes you want to achieve, based on what is most important to you.

Services
CAMHS can provide a service for a child or young person in Inverclyde or Cowal and Bute up to their 18th birthday who may have a moderate or severe mental health problem.
The Adult Community Mental Health Team (CMHT) provides a range of interventions to support treatment, and enable recovery. Access to service is generally via GP referral.
The Primary Care Mental Health Team is a nurse led service providing assessment and follow up for people who have depression, anxiety and adjustment disorders. Access is via GP and or self referral.
The Older Persons Mental Health Service provides a service for those with complex needs arising from dementia, psychosis and more complex or enduring depression and anxiety disorders.
