The service has established a rehabilitation pathway which signposts those seeking help and support to self-management resources as a first option.

The rehabilitation service uses existing referral pathways to deliver Occupational Therapy, Physiotherapy and to support Mental Health.

Intervention strategies are based on initial activity analysis. These include strategies to help patients in understanding physical and cognitive fatigue management, planning and pacing, general strategies for mental health, increasing confidence in self-awareness and self-management, breathing techniques and sleep routine.

Information regarding relevant signposting, as well as support around work and financial issues, is provided.

Referral is provided via the GP.
