Long Term Conditions Hebrides offers:
- activities that can help people living with a Long Term Condition and their carers live better lives.
- Zoom sessions on self-management skills, Mindfulness and Yoga.
- social opportunities, support and encouragement to decrease isolation for people with long term conditions and their carers.
We also provide monthly face to face gatherings and two annual retreats which provide opportunities for alternative or specialist therapies. LTCH liaise and work cooperatively with Local Authority, Health Board, Joint Integration Board, statutory bodies and third sector organisations that support people with long term conditions.
