Care at Home

Glasgow City Health and Social Care Partnership (HSCP) Care Services (also known as Care at Home) provide a range of services and support to help you live independently at home for as long as you can and want to.

If you are assessed as eligible for a service, your care and support will be provided by one or more of the Care at Home teams which include:

  • First Response Team, who look after you when you are discharged home from hospital
  • Reablement Home Care Service for support to get back to doing what you could do for yourself before you were admitted to hospital or had an accident
  • Mainstream Home Care Service for support with personal care, taking medications and preparing food
  • Hot Meal Service (iCare) who provide and prepare meals directly to your home
  • Community Alarms (Telecare) for support with equipment that helps you stay safe at home
