The Community Liaison Service provides 3rd sector support to support hospital discharge and help prevent admissions. We meet with patients. families and carers to offer advice on community supports and refer into these when required.
This support may include: • Social opportunities • Support for families and friends who have a caring role. • Leisure and physical activities. • Future planning. • Support to help you self-manage specific health conditions.
