Community Brokerage Service

We provide independent, 1:1 information, advice and training on Self-Directed Support (SDS), helping people explore their options and make informed choices about their care. We support people to prepare for and take part confidently in social work assessments and reviews, ensuring they feel listened to, informed and less stressed. Through outcome-focused planning, we help people increase their involvement and choice when designing their support, and connect them with relevant community-based services, resources and opportunities.

Our service also builds confidence and skills to manage social care packages effectively. This includes support with budgeting, arranging services, employing personal assistants and understanding responsibilities, alongside access to PA employer training.

Over time, many people feel more confident and independent, with some no longer needing ongoing support to manage their care.
