Connect Me uses simple technology to help you monitor and manage your health at home. In Highland the service is currently being used to support people with Asthma, High blood pressure, Chronic pain, Chronic obstructive pulmonary disease (COPD), Covid-19, and Heart failure with more services being developed and planned.

Connect Me services are free, convenient and easy to use, with a choice of communication methods including text messages, emails and automated phone calls.

Patients are signed up to Connect Me services by their healthcare professional (GP practice or specialist nurse),
